The opportunity

RivMed has an exciting opportunity for an experienced caseworker to join our newly established Aftercare / Leaving Care Team.  The successful applicant will work within a growing, supportive and passionate team at Riverina Medical & Dental Aboriginal Corporation (RivMed) to ensure delivery of the highest quality Aftercare / Leaving Care service. The primary function of the Aftercare Program is to support care leavers, aged 18 to 24, to achieve a successful transition to adulthood.  

The Role

Reporting to the Team Leader – Caseworkers (Aftercare / OOHC) the caseworker will play a pivotal role in supporting our ambitious goal to establish an Aftercare / Leaving Care caseworker service across the Riverina.

The Caseworker – Aftercare / Leaving Care is responsible for providing support and case management to young people transitioning from out-of-home care to independent living. The role focuses on ensuring young people have access to appropriate services, resources, and supports to enhance their well-being and long-term stability. The Caseworker will work closely with young people, their families, and relevant stakeholders to develop and implement transition plans tailored to individual needs.

As a caseworker working closely with our OOHC Team, there will be occasions when you may need to take on case management of an OOHC child, in addition to your responsibilities under the Aftercare Program.  This work will not be considered additional duties but will form part of your general key responsibilities.

Selection Criteria

This is not an Aboriginal identified role however Aboriginal & Torres Strait Islander applicants are strongly encouraged to apply.

  • Preferred qualification, Batchelor of Social Work, Community Services or equivalent. Minimum qualification Diploma of Community Services; or related field; or equivalent relevant skills and experience.
  • Experience working with Aboriginal and Torres Strait Islander communities.
  • Experience working with young people in out-of-home care or leaving care contexts.
  • Strong understanding of the challenges faced by young people transitioning from care.
  • Knowledge of relevant legislation, policies, and best practice frameworks related to leaving care and child protection.
  • Excellent interpersonal and communication skills, with the ability to engage with young people from diverse backgrounds.
  • Strong organisational and case management skills, including assessment, planning, and advocacy.
  • Competency in using client management systems and maintaining accurate records.
  • Demonstrated capacity to network with a range of health, education and welfare services.
  • Excellent verbal and written communication skills including, report writing.
  • Current Working with Children Check and NSW Driver’s Licence.
  • Ability to travel, with possible overnight stays and the ability to work outside of core hours on occasion.
