The opportunity

RivMed has an exciting opportunity for a Program Intake Coordinator.  Reporting to the Clinical Manager, the successful applicant will work within a growing supportive and passionate team at Riverina Medical & Dental Aboriginal Corporation (RivMed) to ensure delivery of the highest quality, services and programs.  

The Role

The Intake Coordinator will play a critical role in coordinating and facilitating client access to a range of health and wellbeing services at RivMed. They will act as the first point of contact for clients and referrers, ensuring that all incoming referrals are screened, processed, and allocated efficiently. This includes monitoring and updating referral waitlists, responding to inquiries, and managing intake appointments. The Intake Coordinator will also attend care discussions and referral allocation meetings, maintain accurate client records, and produce data reports as needed.

In this position, the Intake Coordinator will be responsible for the timely transfer of intake information to the appropriate teams or staff members, ensuring smooth and effective service delivery. Additionally, they will manage urgent referrals according to intake procedures, and liaise with team leaders / managers to ensure clients are onboarded into services promptly and appropriately.

The role also requires adherence to workplace health and safety standards, maintaining confidentiality, and contributing positively to team operations. The Intake Coordinator will engage in ongoing training and development to support their role and RivMeds objectives. This position requires flexibility, attention to detail, and a commitment to providing quality service within RivMed's framework.

The key objectives of the role include, but are not limited to:

  • Conduct Intake screening and referral coordination, including completing relevant assessments monitoring, reviewing, and allocating incoming referrals, ensuring clients are followed up and referrers are contacted in a timely manner.
  • Coordinate, update and review referral waitlists for programs and services
  • Respond to enquiries/requests for information and services related to all mental health and wellbeing services
  • Attend relevant Care Discussion and Referral Allocation meetings
  • Enter information and data into relevant data managements systems in an accurate and timely manner in accordance with guidelines and procedures
  • Maintain up to date client and other records and produce data reports as required
  • Manage and maintain an up-to-date Appointment book for Wellbeing Intakes, ensuring intake appointments are booked in and action as required.
  • Ensure Intake information is transferred to the relevant team/section/staff member and appointments are made to facilitate effective services
  • Ensure that urgent matters are transferred quickly and effectively in line with Intake procedures
  • Answer incoming phone calls and refer incoming calls to the appropriate staff member/team or take accurate messages
  • Liaise with Team Leaders around actioning new client services to ensure positive intake and commencement of service delivery
  • Monitor and evaluate the effectiveness of programs and services, gathering feedback to continuously improve support offerings.

About you

To be successful in the role you will be able to demonstrate you meet the Selection Criteria below:

  • Aboriginality - This is an identified Aboriginal or Torres Strait Islander position and candidates will be required to provide confirmation of Aboriginality with their application. (NB: applicants race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW)
  • Minimum qualification of Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care or similar or relevant experience in a related field
  • Strong understanding of health and wellbeing services, particularly in the context of services offered by RivMed
  • Ability to manage and prioritise multiple tasks
  • Excellent verbal and written communication skills
  • Understanding and respect for Aboriginal culture, with the ability to work sensitively and effectively with Aboriginal clients, families, and communities
  • Flexibility and adaptability to meet changing client needs and organisational requirements
  • High level of integrity, confidentiality, and discretion in handling sensitive client and organisational information
  • Willingness to participate in ongoing training, professional development, and supervision sessions
  • Current Working With Children Check and National Police Check.
  • Valid Driver’s License and willingness to travel for work related purposes.
