The opportunity

RivMed has an exciting opportunity for an Aged Care Connector / Navigator.  Reporting to the Program Coordinator, the successful applicant will work within a growing supportive and passionate team at Riverina Medical & Dental Aboriginal Corporation (RivMed) to ensure delivery of the highest quality, services and programs.  This is a great opportunity for an Indigenous person with an interest in aged care.

The Role

The Aboriginal Aged Care Connector / Navigator (AACN) has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements. 

The overarching function of the AACN is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to RivMed. 

Working closely with the Program Coordinator, the AACN will also help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.  

The key objectives of the role include, but are not limited to:

  • active community outreach to promote aged care services, identifying and engaging with potential clients.
  • support clients to identify goals and needs to ensure they get the appropriate level of care and services, which can include referrals to other specialised services and support. 
  • support clients’ aged care needs through internal referral processes.
  • assist clients with advocacy and support throughout the My Aged Care registration and review processes.
  • provide support to enable client access to appointments.

About you

To be successful in the role you will be able to demonstrate you meet the Selection Criteria below:

  • Aboriginality - This is an identified Aboriginal or Torres Strait Islander position and candidates will be required to provide confirmation of Aboriginality with their application. (NB: applicants race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW)
  • Excellent and strong relationships with the local Aboriginal and Torres Strait Islander Community
  • Good communication and interpersonal skills, with the ability to build trusting relationships in community.
  • Demonstrated understanding of the principles of person-centered care and trauma-informed practice.
  • Experience working with older people.
  • Understanding of the internal RivMed services available to clients.
  • Knowledge of or willingness to learn about the aged care landscape.
  • Computer skills and experience with MS Office.
  • Demonstrated experience working unsupervised and as part of a team.
  • Current drivers licence and willingness to travel for work related purposes.
