Cultural Care Connect Coordinator

This is an Identified role and is only open to Aboriginal or Torres Strait Islander Applicants.

The Opportunity

We are recruiting for a Full-time Cultural Care Connect Coordinator to join the RivMed Mental Health Team.  This is a pilot program so we are looking for someone with the experience to drive this newly established program.

The Role

The Cultural Care Connect Coordinator is a leadership role which will oversee the implementation, delivery and continual refinement of the newly established RivMed Cultural Care Connect Program. The Cultural Care Connect Coordinator will have skills in suicide prevention, grief and loss, crisis support and trauma informed practise frameworks. The role will work alongside individuals and their families to achieve their wellness their goals in a culturally safe environment. Whilst pre-dominantly assisting clients through coordinating and facilitating connections within the community, the Aftercare coordinator will be required to undertake client assessments, co-facilitate support groups for people impacted by suicide and carry a small caseload of clinically supported clients.

Key Responsibilities

  • Support their local Aboriginal and Torres Strait Islander Communities to improve access to culturally safe and appropriate suicide prevention services
  • Establish a suicide prevention network, comprising key stakeholders, Community leaders, Aboriginal and Torres Strait Islander-led and whole-of-population suicide prevention services, other community organisations
  • coordinate the co-design and development of a Network Suicide Prevention Plan, in partnership with community
  • participate in relevant Communities of Practice, established to support ACCHOs in undertaking service activities, and facilitate sharing of best practice and expertise
  • Undertake reporting on CCSPN activities in line with Service Agreement requirements
  • Participate in evaluation and monitoring activities, as described in NACCHO’s Monitoring and Evaluation Framework, including ensuring appropriate data collection.
  • Providing consultation to staff and managers to assist in the decision making process on psychological, developmental, and emotional and behavior management issues.
  • Delivering training to staff, families, carers, foster parents and external agencies.
  • Maintaining electronic clinical records and provide written reports.
  • Participate in regular supervision with your supervisor
  • Liaise with community and health linkers to provide optimal services for clients

Selection criteria

This is an Aboriginal identified position and is open to Aboriginal & Torres Strait Islander applicants only – please supply a copy of your Aboriginality along with your Resume and Cover letter.

  • Certificate in Mental Health or Aboriginal and/or Torres Strait Islander Primary Health Care OR equivalent training or minimum two years' experience within the field
  • Confirmation of Aboriginality
  • Demonstrate a passion for supporting Aboriginal and Torres Strait Islander community
  • Excellent written and oral communication skills
  • Demonstrated understanding of relevant legislation and policy frameworks
  • Client focused with an ability to advocate on the client's behalf
  • Ability to multi-task, prioritise workloads and meet deadlines
  • Proficiency in computer software including the Microsoft Office suite 
  • Commitment to personal and professional development including the sharing of knowledge, information and skills with work colleagues
  • A current working with children’s check clearance, National Criminal check and drivers licence.
