Culture Care Connect - After Care Worker

This is an Identified role and is only open to Aboriginal or Torres Strait Islander Applicants only.

The opportunity

We are recruiting for a full-time After Care Woker to join the Riverina Medical and Dental Aboriginal Corporation. 

The Role

The After Care Worker will help improve Aboriginal and Torres strait islander mental health and suicide prevention outcomes by providing culturally appropriate aftercare service and support to the community.  The After Care Worker will work alongside individuals and their families to achieve their wellness goals in a culturally safe environment.

Key Responsibilities

  • Support their local Aboriginal and Torres Strait Islander Communities to improve access to culturally safe and appropriate suicide prevention services.
  • Contribute to a localised service delivery model for aftercare services through a co-design approach with local Community, using NACCHO’s National Model of Care, and other resources, as a framework.
  • Provide culturally safe aftercare services to Aboriginal and Torres Strait Islander individuals who have been referred following a suicide attempt or suicidal crisis in line with agreed service delivery model
  • Build and maintain strong referral pathways, including but not limited to the following services: acute care services, primary care services, schools, police, funeral services, Alcohol and Other Drug Services, mainstream and Aboriginal and Torres Strait Islander-led mental health services
  • Maintaining electronic clinical records and provide written reports.
  • Participate in regular supervision with your supervisor
  • Liaise with community and health linkers to provide optimal services for clients.

Selection criteria

This is an Aboriginal identified position and is open to Aboriginal & Torres Strait Islander applicants only – please supply a copy of your Aboriginality along with your Resume and Cover letter.

  • Certificate in Mental Health or Aboriginal and/or Torres Strait Islander Primary Health Care OR equivalent training or minimum two years' experience within the field
  • Confirmation of Aboriginality
  • Demonstrate a passion for supporting Aboriginal and Torres Strait Islander community
  • Excellent written and oral communication skills
  • Demonstrated understanding of relevant legislation and policy frameworks
  • Client focused with an ability to advocate on the client's behalf
  • Ability to multi-task, prioritise workloads and meet deadlines
  • Proficiency in computer software including the Microsoft Office suite 
  • Commitment to personal and professional development including the sharing of knowledge, information and skills with work colleagues
  • Current and clear Criminal Record Check, NSW Working with Children Chek and current Drivers licence
